The Adult Foster Care (AFC) program is a MassHealth Program that provides personal care services in the home to elderly or disabled individuals age 16+ who are no longer able to live alone and want to avoid an institutional setting. A Caregiver is usually a family member or close friend selected by the client. Both the Client and Caregiver must reside in the qualified setting and the client must have their own bedroom.
