Simplifying Care: A Seamless Admission Process with Community Connections Healthcare

When it comes to receiving care at home, the process should feel supportive—not overwhelming. At Community Connections Healthcare, we understand that families and individuals are often navigating important decisions during stressful times. That’s why we’ve designed our admission process to be simple, efficient, and handled with care from start to finish.

Getting started is easier than most people expect.

Whether you’re calling in directly or being referred by a healthcare provider, the first step is completing a quick referral form. This initial step is designed to gather the basic information needed to begin the process—nothing complicated, nothing time-consuming.

From there, our Admissions Department takes the lead.

Once the referral is received, our experienced team coordinates everything on your behalf. This includes communicating with physicians, arranging any necessary appointments or documentation, and ensuring all required steps are completed for program eligibility. Whether it’s for MassHealth’s Group Adult Foster Care (GAFC) or Adult Foster Care (AFC) services, we guide each case through the process with precision and attention to detail.

Families don’t have to chase paperwork or manage logistics—we do it all.

Our goal is to remove the burden from consumers in need and their loved ones. Instead of navigating multiple systems, making countless phone calls, or worrying about what comes next, you can rely on our team to keep everything moving forward smoothly.

We stay with you every step of the way.

From the moment a referral is submitted to the point of approval for services, our Admissions team remains actively involved. We provide updates, answer questions, and ensure that nothing falls through the cracks. This level of support allows clients to focus on what truly matters—receiving quality care in the comfort of their own home.

At Community Connections Healthcare, care doesn’t just begin when services start—it begins the moment you reach out.

If you or a loved one is considering in-home care, know that the process doesn’t have to be complicated. With a dedicated team managing the details behind the scenes, getting started is simple, stress-free, and designed with your peace of mind in mind.
