In the home health agency industry, CARF accreditation is widely recognized as the gold standard for delivering best-in-class services to consumers in need. CARF is an international organization founded in 1966 with a mission to promote the quality, value, and optimal outcomes of services through a consultative accreditation process and continuous improvement initiatives that center on enhancing the lives of the persons served.

At Community Connection Healthcare, CARF standards serve as the guiding principles for how we operate every day. Our systems, policies, and procedures are designed to protect the health and safety of the individuals we serve while supporting and empowering our employees to deliver the highest quality of care. This approach allows the consumers we support to live independently in their own homes and remain active members of their communities.

We continuously review and update our policies and procedures to stay responsive to market changes, evolving state regulations, and the needs of both our clients and staff. Open communication is a cornerstone of our culture. We maintain regular communication with our direct care aides, nurses, and care managers to ensure they feel supported and informed in their roles.

To maintain excellence in care delivery, we provide ongoing training and conduct in-person competency skills assessments. This ensures our direct care aides consistently provide safe, proper, and personalized care tailored to each consumer’s unique needs.

The CARF survey process occurs every three years and includes an in-depth review of our business practices, policies, procedures, and training programs. Community Connection Healthcare has a dedicated Compliance Officer and a team of staff committed to the CARF accreditation process, serving as a system of checks and balances to ensure we not only meet—but exceed—industry standards.
