Finding quality in-home care services for yourself or a loved one can feel overwhelming. Between paperwork, medical documentation and appointments and navigating MassHealth requirements; many families and healthcare professionals worry about how complicated the process may become. At CCH, we work to make admission into AFC and GAFC services in Massachusetts as simple and stress-free as possible.
Whether you are a family seeking support, a hospital discharge planner, or a case manager helping a client transition safely back at home and into the community, our team is committed to guiding every step of the process.
What are AFC & GAFC Services?
Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) are MassHealth caregiver programs designed to help individuals remain safely in their homes or community settings while receiving assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs). **For AFC services, consumers must live with a qualifying caregiver.
These services may include support with:
● Bathing
● Dressing
● Mobility
● Toileting
● Eating
● Meal Preparation
● Grocery Shopping
● Light housekeeping
● Medication Reminders
Making the Admission Process Easier:
One of the biggest concerns families and referral sources have is how long and difficult the enrollment can be. At Community Connection Healthcare, we simplify the process by handling nearly everything electronically and coordinating directly with providers and caregivers.
Our team manages :
● Intake Coordination
● Communication with Primary Care Providers (PCP’s)
● Tracking required paperwork
● Scheduling assessments and nursing visits
● Following up with doctor’s offices
● Assisting caregivers with required documentation
In most cases, the client and family only need to stay current with medical appointments, including annual physicals and follow-up visits. We take care of the rest.
What Does the Admission Process Look Like?
